In most countries, obtaining consent before treatment is essential. Clinicminds lets patients sign consent forms digitally before a treatment is performed.
Preparing your content
The required content of a consent form depends on local regulation and legislation, so Clinicminds cannot provide example content. We advise you to request digital copies of consent forms from the suppliers of the products you work with, or from your professional association, and adapt that content to your own services.
Create a consent form
Click the plus (+) icon in the top right corner.
Under Treatment categories, select the categories this form applies to. Use Select all or Deselect all to speed this up. The form is only shown at appointments and records of the selected treatment categories.
In the Name field, enter a clear name, for example "Informed consent form - Tattoo removal".
In the Content editor, paste or write the text of your consent form. Use "paste and match style" when pasting, so the text takes over the Clinicminds formatting. The editor supports bold, italic, underline, text sizes, and bulleted and numbered lists.
Click Save.
Edit or remove a consent form
Click an existing form in the list to open it. Adjust the categories, name, or content and click Save, or click Remove to delete the form.
Note: adding and editing consent forms needs to be performed on a PC/Laptop and not on an iPad.
