To add headers and text blocks to medical questionnaires navigate to Menu > Record settings > Medical questions.
Create the plus in top right corner start by adding a header (e.g. allergies). Next, scroll down and press save.
If you desirable, you're able to create add a text block. Click the plus icon in the top right corner. Select text and write down the explanation which you wish to display below the header (e.g. This next section will allow us to see if there are any potential risks with pre-existing medication).
Finally scroll down and press save.
You can move the rows up and down by clicking on the far right double arrows and dragging them to the desired position.
