After finishing the case history and medical state, continue to the forms tab. Select the applicable informed consent, corresponding to the treatment that will be performed. It is possible to select multiple consent forms. 

Next click the ‘sign’ button.

Tip: When using an iPad, you are able to hand it over to the patient in order to sign the form, for example using a stylus (e.g. Apple Pencil).

Next hit the save button to see a confirmation of the signed form.
