We’ve modernized the design of patient forms to make them mobile-friendly, pleasant to use, and consistent with your clinic’s brand. Patients can now complete their forms more easily on any device, while you ensure the look matches your clinic’s style.
You can find these settings under: Menu > Clinic settings > Clinic > Branding.
Which forms are affected?
At this time, the branding settings apply to the medical questionnaire form.
This form includes two parts:
Personal data section – where patients provide their personal details.
Medical history section – where patients answer questions about their medical background before their visit.
The medical questionnaire is typically sent to patients after booking an appointment manually (by phone or in person) or when they book online, if you select the option “Let patient submit data”.
What can you customize?
You can personalize the appearance of the medical questionnaire using the following settings. A live preview is shown on the right-hand side of the screen, so you can instantly see how your changes will look before saving.
Title Font style – affects the "Welcome" text at the top of the form (note: the text itself cannot be changed). Font style options include: Classic, Neutral, Minimalist, Professional, and Handwritten.
Header color – changes the color of the form header, where your clinic logo is displayed.
Button color – affects the button at the bottom of the form (used to continue or submit). This color setting also applies to the welcome text at the top of the page. You can use the color picker or enter a color code in RGB, HSL, or HEX.
Button style – choose Sharp, Rounded, or Circular. This style also applies to input elements like answer fields and selection buttons throughout the form, as well as the button at the bottom of the page.
Once you are happy with the result, don’t forget to save your changes to apply them to the patient form.
